Band 7 Community Discharge Service Coordinator

Job Description

  • The post holder will work collaboratively with the multidisciplinary / multiagency health and social care teams within the acute hospital, community hospital and community teams to co-ordinate and navigate the patient across discharge pathways to ensure high quality person centred service and ensure safe, effective and timely discharges.
  • The post holder will ensure that no long term decisions about future care are made in hospital and will assist in developing further the ‘Discharge to Assess’ ethos. The post holder will ensure that ‘Home First’ is embedded across the service utilising the existing pathways through Intermediate Care. If discharging home is not an option the post holder will consider other discharge options including in-patient rehabilitation, bed based Rehab and interim beds to enable a period of assessment and care planning to determine long term need, clearly evidencing rationale for any provision of bed based care.
  • The post holder will communicate on-going care needs to relevant hospital teams, Intermediate Care, Social Worker teams, Community Nursing, Independent /Statutory sector, Community/Voluntary bodies, users and carers, to provide the best outcomes and promote a home first approach at all times.
  • The post holder will highlight patients requiring complex discharge planning, review the complex delays daily and participate in the update of complex coding / data.
  • The post holder will liaise closely with HSW in managing the complex patients to ensure safe D/C from hospital in a timely manner.They will be responsible for the delegation of appropriate tasks/duties to relevant staff and manage complex discharges throughout the hospital and community.
  • The post holder will be responsible for providing service performance data that will illustrate the efficiency and effectiveness of the service, highlight areas for improvements, and take responsibility for implementation.
  • The post holder will be responsible for complying with robust governance frameworks and protocols to ensure safe discharges and high quality care.
  • The post holder will be required to undertake flexible working to meet the needs of the service that will include extended seven day working including weekends & bank holidays.
  • The post holder will provide an in reach function to acute hospitals sites and be responsible for coordinating and ensuring effective complex discharges within the Belfast trust

Essential Criteria

  •     5 years post registration experience With a Minimum of 3 years clinical/ managerial experience at Band 6
  •     Demonstration of experience in discharge planning from Acute care episode or facility
  •     Professional Qualification registered with a specific professional body within Nursing(NMC) / Occupational Therapy/ Physiotherapy (HCPC) or Social Work (NISCC).
  •     Hold a current full driving licence which is valid for use in the UK and have access to a car
