Hospital Discharge Coordinator - Social Work/Nursing/OT

We are recruiting for a Health Care Professional for the Hospital Discharge Hub for an initial 3 months period based in the Royal Victoria Hospital.  It will be computer based and will require the candidate to be fully computer & phone literate.

The successful candidate(s) will be required to have a minimum 3 years post qualifying experience in a similar role.  Applicants from Nursing, Physiotherapy, Occupational Therapy, Social Work and all other professions will be considered, providing they have the relevant experience in Adult & Social Care.

This will ideally be a full time role, however part time hours may be considered for the right candidate(s).
