Social Work Assistant - Hospital Services

I have two Social Work Assistant positions coming available in Hospital Discharge Services across two sites in Belfast. 

 

You will work within the Social Work Team to ensure safe, accurate and efficient discharge from Hospital to the Community or Residential Care.  You will be a first point of contact for ongoing assessment and support for patients and their families. You will undertake case work and assessment as required and maintain accurate patient records.  This is a busy team that engages with multiple disciplines across the Hospital and Community. 

 

Post 1: Royal Victoria Hospital - Working on a range of medical and surgical care wards

Post 2: Meadowlands, Musgrave Park - Working with long stay patients with traumatic brain injury 

 

Both Teams have excellent reputations within the Trust and the posts are coming vacant due to applicants moving to another sector outside the NHS.  

 

These are long term temporary with hours set Monday to Friday 0900 to 1700.  Salary set £12.25ph. 

 

Ideally we would prefer candidates to have 6 months adult or social care services experience and be NISCC registered.  Hospital experience would be beneficial.  Ability to communicate calmly during crisis and advocate for patients and families is essential.
